This document explains what telehealth is, what to expect from a telehealth visit at Elevate Mental Health Clinic, PLLC, and your rights and responsibilities. By scheduling and attending a telehealth visit with us, you acknowledge that you have read, understood, and agree to the terms below.
“Telehealth” or “telemedicine” means the delivery of healthcare services using interactive audio, video, or other electronic communications technology between a healthcare provider and a patient who are not in the same physical location. Telehealth at Elevate is typically conducted via real-time secure video over a HIPAA-compliant platform. In limited circumstances, telephone-only visits may be appropriate.
Bonny Benjamin Petty, PA-C, our licensed Texas physician assistant, provides psychiatric evaluation, medication management, and supportive psychotherapy via telehealth to patients located in Texas at the time of the visit. Both new-patient (initial) and follow-up visits are available.
All information disclosed during telehealth visits is treated with the same confidentiality protections that apply to in-person visits, including under the federal Health Insurance Portability and Accountability Act (HIPAA) and Texas Health and Safety Code Chapter 611. The same exceptions to confidentiality apply (mandatory reporting of suspected abuse, imminent danger to self or others, court orders, etc.). You must be physically located in Texas at the time of each telehealth visit. Bonny is licensed to provide care only to patients located in Texas. If you will be outside Texas at the time of a scheduled visit, please notify us in advance so we can reschedule. Providing care across state lines without proper licensure is prohibited by law and we will not knowingly do so.
At the start of each visit, your clinician will confirm your location and verify your identity.
We do not record telehealth visits. You may not record visits without the explicit written consent of your clinician.
Where clinically appropriate, your clinician may prescribe medications by telehealth in compliance with federal and Texas law. Controlled substances are subject to additional restrictions and may require an in-person visit. Your clinician will discuss any prescription decisions with you.
You have the right to refuse telehealth at any time and to request an in-person visit at our Georgetown office instead. You may also withdraw consent for telehealth services at any time without penalty, although doing so may affect your ability to receive certain services.
Telehealth visits are billed at the same rates as in-person visits. Most insurance plans cover telehealth services for behavioral health; coverage may vary by plan and is not guaranteed. We will help verify coverage but you are responsible for any amounts not covered by insurance.
If you are experiencing a psychiatric or medical emergency, including thoughts of suicide or self-harm, call 911 or go to your nearest emergency room immediately. You may also call or text the 988 Suicide and Crisis Lifeline (24/7) or text HOME to 741741. Telehealth visits are not a substitute for emergency care.
At the start of treatment, your clinician will discuss a safety plan with you, including local resources and contacts to use in case of emergency.
