A clinical relationship with Elevate Mental Health PLLC is established only after you have completed our intake process and been seen by one of our licensed clinicians for an evaluation appointment.
If you are experiencing a medical or psychiatric emergency, including thoughts of suicide or self-harm, call 911 or go to your nearest emergency room immediately. You may also call or text the 988 Suicide and Crisis Lifeline (24/7) or text HOME to 741741 for the Crisis Text Line. Do not use this Site, our chat, our SMS line, or our voicemail to communicate emergencies — those channels are not monitored in real time. SMS is not for emergency or crisis care.
You must be at least 18 years of age to use the Site on your own behalf. Parents or legal guardians may use the Site on behalf of a minor in their care.

Messages may include appointment scheduling, appointment reminders, responses to inquiries, and clinic communications. Message frequency varies. Message and data rates may apply. You agree to use the Site and Services only for lawful purposes and in a manner that does not infringe the rights of any third party, violate applicable law, interfere with or disrupt the Site, attempt unauthorized access, or transmit malicious code.
Email is not a fully secure form of communication. Do not send sensitive medical information by email. We may respond to email inquiries but cannot guarantee confidentiality of the content you transmit.
Our website may include a chat widget. Conversations through that widget may be reviewed by AI-assisted tools and human staff for service and quality. Do not share sensitive medical information through the public chat — please call or schedule a visit for clinical conversations.
Appointments may be booked through our website, by phone, or through our staff. We request at least 24 hours’ notice for cancellations or rescheduling. Late cancellations and no-shows may be subject to a fee per our practice policies, which you will receive at intake.
We are in-network with select insurance carriers. The current in-network list is published on our website and is subject to change. If we are not in-network with your plan, we offer self-pay rates and will file claims with your insurance carrier on your behalf so that you may pursue out-of-network reimbursement. Payment is due at the time of service unless other arrangements are made in writing.
All content on the Site is the property of Elevate Mental Health PLLC or its licensors and is protected by U.S. and international copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works of any Site content without our prior written permission, except for personal, non-commercial use.
